Thank you for your interest in having us provide services to you or your client/patient. We value the importance of coordinated care and a strong provider/clinic relationship.

 

New Client Appointments

Please complete the form above so that we may begin the outreach process. Typically, we strive to get new clients scheduled within two weeks or less from the initial referral. However, due to our popularity, we can experience times of which the client may need to be put on a wait list (generally no longer than 4 weeks). If we are unable to get your client scheduled within a reasonable timeline, we will do our best to notify you so you can ensure that they are receiving the support they need in the meantime.

 

Support Services

We are passionate about serving our community by providing a unique, professional, and thoughtfully designed experience. We take a collaborative approach with the client and match them with the skills, experience, and expertise of our talented staff.

 

Treatment Timeline

The treatment timeline is unique to each individual. Typically however, we estimate that most clients can expect a minimum of 12 weeks of therapeutic support with a check in and treatment plan update at their 6 week visit. Towards the 12 week mark, we continue to work with the client to identify if continued support is needed or not. This can also be determined by the client’s insurance carrier.
